The purpose of this funding announcement is to provide technical assistance to HRSA grantees and other providers of healthcare services by supporting the Healthcare Communities Knowledge Gateway, a robust knowledge and content management system and related quality improvement efforts that bridge healthcare,

public health, local and state based community organizations in improvement efforts across the United States.

The websites facilitate community based efforts by organizations to improve healthcare systems and related patient health and outcomes.

HCKG provides support to the following:
Patient Safety and Clinical Pharmacy Services Collaborative participant organizations and related community based activities; the Donation and Transplantation Community of Practices continued growth and success in saving lives and increasing awareness of the need for additional donors; and sharing of best practices and other resources for the State Health Access Program.

Leaders from Organ Procurement Organizations, Transplant Centers, and hospitals use the website to share best practices, tools and resources that will assist in increasing organ donation and success in transplants.
