Specifically, the purpose of

credit:

this FOA is to implement and test the ability of a model of clinical childhood obesity management along with a EHR-referral-based, family-centered pediatric weight management program, to reduce body mass index (BMI) in children with obesity, or overweight with risks (including, e.g., medical and behavioral risks, and family history).

Health outcomes of interest include measurable changes in BMI, nutrition and physical activity behaviors, health care satisfaction and service delivery, and quality of life.

This will be accomplished through optimized pediatric care:1) Implementation and evaluation of a EHR-referral-based, structured, intensive family-centered weight management program in the clinic or community; 2. Collaboration with state CHIP offices to advise a state-wide or regional level project and to be part of a stakeholder group that can help generate suitable recommendations for sustainability and program components to be further replicated or scaled.
