Additional Information of Eligibility:
Eligible applicants are those entities that have received a grant under sections 736,739 or 747 of the Public Health Service (PHS) Act programs.  Section 736 includes the HRSA Centers of Excellence program; section 739 includes the Health Careers Opportunity Program; and section 747 includes the Primary Care Training and Enhancement programs.  The Primary Care Training and Enhancement programs include the Academic Administrative Units in Primary Care, Interdisciplinary and Interprofessional Joint Graduate Degree,  Physician Assistant Training in Primary Care, Physician Faculty Development in Primary Care, Predoctoral Training in Primary Care, Residency Training in Primary Care, Primary Care Residency Expansion Program, and the Expansion of Physician Assistant Training programs.

 Applicants must provide documentation of prior grants under these sections in Attachment 7.

Foreign entities are not eligible for HRSA awards, unless the authorizing legislation specifically authorizes awards to foreign entities or the award is for research.  This exception does not extend to research training awards or construction of research facilities.
